We have eliminated much of the initial material about preparation and setup. This is easy to explain live, and most learners will already have lots of experience doing PAP smears, so information about positioning in the stirrups etc is superfluous.
1. In the first segment below, the link takes you directly to speculum insertion, purpose of the tenaculum, and insertion of the uterine sound. We excerpted this from the Brookside material, originally found here on YouTube:
We tuned out the background music that comes with the original video!
This second short section shows the IUCD being inserted. It goes in quite easily and should not be forced:
In our opinon, the original Brookside video segment mentioned above, while nice and clear, skips through the device insertion steps quite quickly. Also it is based on the Copper T device, which is much less commonly used in our region than the Mirena device.
2. We excerpted the next segment below from this animation about the Mirena device. There is no audio in this next segment.
3. This next section from a different video shows how to use the tenaculum:
4. Later, in this same video, there is more information about the specific steps for a Mirena insertion:
Finally, we return to the Brookside video in the next segment below to watch the actual IUCD insertion. Perhaps these rapidly performed steps will make more sense now. But remember that the device used by Brookside is different so the exact actions will also be different. Refresh your memory with the product manual for each device before starting - you don't want to get half way through and then have to pause.
One other small point. In our opinion, the original Brookside video advocates for cutting the string shorter than we usually recommend. We prefer the 4cm recommended in the Mirena animation in the second segment.
Remember to give the patient advice about checking for device prolapse and expulsionafter each period. (This is not mentioned in any of the videos we found, here or elsewhere on the Internet.)
Thanks for watching.
